The following recorded webinars are available. They were originally provided as training for SPNS grantees associated with the Medical Home HIV Evaluation & Resources Team (Med-HEART) and are intended for staff such as patient navigators who work to help clients who are homeless find and retain housing.

This recorded webinar describes the components of housing advocacy, approaches to housing for people who are homeless and have disabilities, and examples from three SPNS grantees of how they approach the housing needs of their clients. James Apt, Boston Health Care for the Homeless Program, led the discussion. Katie Hill, PATH – People Assisting the Homeless; Verna Gant, Family Health Centers of San Diego; and John Pauly and Carl Falconer, River Region Human Services, Inc., presented with James.

Pre-Housing Considerations for Navigators

Looking for housing is a stressful process even when everything goes smoothly. And it doesn’t end on move-in day. James Apt, Case Manager at Boston Health Care for the Homeless Program, describes some of the many considerations he keeps in mind when working with a client on housing needs. James presents lots of recommendations for some of the up-front discussions navigators may want to have with clients to ensure that expectations are set correctly and the relationship with the client builds on a foundation of trust.

You’re Housed, Now What?

James Apt, Case Manager at Boston Health Care for the Homeless Program, presented considerations and strategies for keeping formerly homeless clients on track once they are housed.
Please note: This webinar gets off to an abrupt start on the first slide where James presents points to discuss with clients who have just moved into housing. The webinar was presented to grantees of the Med-HEART project and contains some references specific to that project, but we are making it publicly available because the main content is useful to those who work to house clients who are homeless and may be multiply diagnose with HIV and mental health or substance use issues.

James Apt and Matthew Mitchell, Case Managers at Boston Health Care for the Homeless Program, talk about how they incorporate harm reduction into their work with clients who are housed to work as a team to accomplish the behavioral change goals that clients set for themselves, such as paying rent on time or finding a daily routine when they are newly housed.
