Health and Housing:Partnering at the Agency and Systems Levels

New programs across the country are showing the benefits of health centers and supportive housing providers working together to best serve patients who are frequent users of emergency health services by providing them with housing and services, which results in improved lives and lower costs to the health care system. To best serve this population, the health and housing sectors must establish strong, mutual partnerships with common goals and clear roles. In this webinar you will hear from CSH and health centers that have lead the way in partnering with housing providers. Different geographic regions and types of partnerships will be represented, and the discussion will center on how to establish and maintain these partnerships as well as how health centers can benefit from them.
