Project Title: SPNS HIV + Oral Health Program and Evaluation (HOPE) Project

Organizational Setting:

The Native American Health Center (NAHC) is a federally qualified Health Center that operates three clinics in the San Francisco area.

Goals/Objectives:

The HOPE Project will establish the NAHC as the “dental home” for HIV+ San Franciscans of color. HOPE will improve access to oral health services for HIV+ individuals, deliver culturally competent dental case management to HIV+ Native Americans and Hispanics/Latinos, and build capacity in the dental safety net for HIV+ patients.

Primary dental care will be offered through NAHCs existing on-site structure. Specialty services will be provided off-site in partner specialty dental offices.

Target Population:

Persons of color who are HIV+. The HOPE Project will target individuals who are Native American, Hispanic/Latino and individuals who are multi-ethnic. There are 26,110 San Francisco residents diagnosed with HIV/AIDS, and only 2000 of these individuals were served in Ryan White funded dental clinics. The HOPE Project will target difficult to reach patients.

Program Intervention/Services:

The project will provide 250 patient visits to 40 unduplicated patients. One fourth of visits will occur in private practice specialty dentists’ offices with the remainder, including dental case management and primary dental care, provided at the Native American Health Center.

Program Staff:

The HOPE Project is staffed by a dental director, clinical and administrative directors, staff dentists, a patient navigator and a dental assistant. Dental specialists will be included through sub-contracts.

Partners & Linkages to Medical Care:

The project will have sub-contractual linkages with dental specialists in the community. In addition, they will collaborate with two new partners for referral services; a Latino mental health agency and a substance abuse rehabilitation program. Ongoing linkages include other NAHC providers and the San Francisco Community Clinic Consortium member health clinics.

Consumer Involvement:

NAHC has an ongoing consumer committee and consumers were involved in developing the SPNS proposal.
