Project Title: Treating, Educating and Evaluating for Total HIV Oral Health (TEETH OH)
Organizational Setting:
Montefiore Medical Center (MMC) is a major teaching hospital in New York City.
Goals/Objectives:
To improve the delivery of oral health services to the HIV+ population in targeted communities by designing and implementing an integrative model of dental and medical primary care. To increase overall usage of oral health services in the HIV+ population, and utilize the RE-AIM framework to evaluate the effectiveness of the model.
Geographic Setting:
Urban; mobile van.
Location:
On-site at three hospital-affiliated Community Health Centers and through a mobile van at seven other MMC community-based primary care sites throughout the Bronx, New York.
Target Population:
1000 HIV+ patients and their family members who currently receive health services at MMCs ten community based primary care facilities in the Bronx. A reported 11% of these patients are currently receiving oral health care.
Program Intervention/Services:
TEETH OH will integrate HIV services and dental services provided through MMC’s Medical Group. Dental hygienists will be integrated into ongoing Ryan White CARE Act funded support teams.
Program Staff:
TEETH OH is directed by the Ryan White Title III funded program director. There is also a dental director and a medical director. Dental services will be provided primarily by a newly hired bilingual dental hygienist who will work with primary care teams at each site.
Partners & Linkages to Medical Care:
TEETH OH will be integrated into ongoing primary care sites. Oral health services will be provided on site at three health center locations. At seven locations, oral health services will be provided through a mobile van unit.
Consumer Involvement:
The project will be affiliated with an active and ongoing Consumer Advisory Board.
