Bridging the gaps for families of children with special health care needs
The Rhode Island Pediatric Practice Enhancement Project (PPEP) places and supports trained Parent Consultants in pediatric primary and specialty care practices. They work as part of the medical home team by assisting families and providers in accessing community-based resources and specialty services. They also help identify barriers to effective care coordination.
This nationally recognized program is a model for cost-effective, parent/professional partnership in the medical home and provides a real-life illustration of the inter-connectedness of the six Healthy People 2010 performance outcome measures for children and youth with special health care needs.
