Disease Management Programs
Wyoming Medicaid has disease and health management programs that assist people with chronic illnesses, such as diabetes or asthma, as well as obesity, serious emotional disturbance, and high-cost or serious health problems. These disease management programs make use of telemedicine. They also use a “health buddy,” which is an interactive, automated phone interview system that poses a series of questions to the user. It provides feedback on user responses and links him or her to a case manager if a need for intervention is indicated. Families without phones may receive free phones and phone service to interact with the health buddy.
